The medical quality level shows “four liters and one dropâ€
The so-called "four liters", first, the supply of medical resources continues to increase. In 2017, the total number of medical practitioners in the national medical and health institutions was 8.18 billion, an increase of 3.2% over 2016; the number of discharged people was about 244 million, an increase of 7.5% over 2016. Second, the quality of some specialties, key diseases and surgical diagnosis and treatment has risen steadily. From a specialist level, the mortality rate of the low-risk group in traditional high-risk surgical departments such as thoracic surgery, cardiac vascular surgery, and neurosurgery decreased from 0.16%, 0.15%, and 0.14% in 2013 to 2016. 0.06%, 0.11%, and 0.04%. From the key diseases and surgical level, 16 key diseases monitored by gastrointestinal hemorrhage, chronic obstructive pulmonary disease, cerebral hemorrhage and cerebral infarction, and 13 key surgical diagnosis and treatment indicators for craniocerebral surgery and heart valve replacement were stable. In the middle, the mortality rate of inpatients in 16 diseases continued to decline. Third, the level of clinical rational drug use continues to increase. Taking antibacterial drugs as an example, the use rate of antibacterial drugs in outpatient clinics in China dropped from 19.4% in 2010 to 7.7% in 2017, a decrease of 11.7 percentage points. The use rate of antibiotics in hospitalized patients decreased from 67.3% in 2010 to 36.8% in 2017, a decrease of 30.5 percentage points. The trend of bacterial resistance is generally stable. Fourth, the efficiency of medical services has improved.
The so-called "one drop" is that the mortality rate of inpatients continues to decline and is stable at a lower level. The average hospitalization days for tertiary and secondary hospitals in 2017 were 9.8 days and 8.7 days, respectively, down 0.3 days and 0.1 days respectively from 2016, achieving a five-year continuous decline. The total mortality rate of inpatients in tertiary public hospitals decreased from 0.74% in 2014 to 0.67% in 2016, achieving a three-year decline.
Specialist development and medical technology capabilities show "two balances, four upgrades"
According to Guo Yanhong, during the “Twelfth Five-Year Plan†period, the state finances invested a total of 6 billion yuan, supporting 79 majors and 1231 clinical key specialists in 314 hospitals. The provinces have also increased their investment in the construction of clinical specialties in provincial and county-level medical institutions. Through continuous support and construction, China's specialist development and medical technology capabilities have shown a benign posture of “two balances and four enhancementsâ€.
"Two balances" , first, the inter-regional technical capabilities are more balanced. The regional synergy ability has been enhanced, the quality medical resources of the specialists have been effectively and orderly sank, the technical capabilities of the central and western regions and county hospitals have been greatly improved, and a number of intra-regional technical gaps have been filled. Second, the development of specialties is more balanced. Platforms such as anesthesia, critical medicine, and pathology, as well as pediatric, emergency, mental, and rehabilitation, have developed rapidly. The gap in technical capabilities between specialists has narrowed significantly, and the coordinated development of specialists has gradually formed. Taking pediatrics as an example, the construction of national key clinical specialties during the “Twelfth Five-Year Plan†period has given great support to the disciplines of pediatric intensive care, pediatric digestion, pediatric respiration, newborn, children's oral and pediatric surgery, and promoted the effective improvement of pediatric specialist ability.
"Four promotion" , first, the clinical service capacity has improved significantly. The average number of specialized diseases such as pain, oral and maxillofacial surgery, rehabilitation medicine and organ transplantation increased by more than 10%, and the average increase in rehabilitation medicine and organ transplantation was over 20%. The professional CMI index (case combination index, representing the difficulty of treating diseases) in neurosurgery, infectious diseases, burns and pain departments increased by more than 10% on average, and the average increase in burns and pain departments was over 20%.
Second, major breakthroughs were made in key diseases and key technology areas. The technological innovation and transformation capabilities have been further enhanced, resulting in a number of superior medical technologies that have reached or led the international advanced level and have demonstrated and acted internationally. According to incomplete statistics, various specialists have applied for 7,789 patents of various types, resulting in a number of superior medical technologies representing the highest level in China and even in the world. For example, the ventricular assist device independently researched and developed by the Chinese Academy of Medical Sciences Fuwai Hospital has carried out clinical application, solving the difficult situation of long-term ventricular assisted “no pump available†in China, and filling the technical gap in the domestic ventricular assisted research and development field.
Third, the international influence of the specialists has been significantly enhanced. The “Chinese Voice†of the medical industry is constantly recognized by international counterparts. According to incomplete statistics, 3812 people have served or have served as members of the major international academic organizations, journals, magazines or editorial boards of the college. 340 people took the lead or participated in the development of the international guidelines, norms and industry standards. The research results of Renji Hospital of Shanghai Jiaotong University on gastrointestinal vascular disease became the sole basis for the 2015 American Gastroenterology Association guidelines.
